How to Enroll

New employees may enroll in the Term Life and Disability Program within 30 days of their date of hire without any medical questions being asked.   

You may also enroll in the Life and Disability Program any time during the year by completing an application, however, a medical history statement must be submitted to the insurance carrier (The Standard) for their approval.
